Our Parent Guide has information that you'll need to know to prepare before the first day of camp.
Forms
Please log in to your UltraCamp account to ensure all camper forms are complete no later than two weeks before the start of your camp session.
Immunization Form
We ask that you submit your child's immunization record on this form.
medication
If your child will need to have medications administered while at camp please complete and upload this form to your UltraCamp account, this includes emergency medications like epinephrine and inhalers.
If your camper will need an Epinephrine Auto-Injector while at camp please complete and upload this form to your UltraCamp account in addition to the medication form.
If your camper will need an inhaler while at camp please complete and upload this form to your UltraCamp account in addition to the medication form.
camper physical for Adventurers excursion
If your camper is attending the Adventurers Overnight Excursion please complete and upload this camper physical exam form to your UltraCamp account.
